In Addition, the last page of the 2020-2021 Parent-Student Handbook (below) must be signed and returned along with all forms above.


HOW TO RETURN THE FORMS

Option 1:
Return all completed and signed forms as an attachment via email to MayCruz@dadeschools.net
**Include student name and ID number in the SUBJECT LINE of your email**

Option 2:
Return completed and signed documents in an envelope to the school Dropbox located at the security desk at the main entrance of the building available from 8:30am – 3:00pm.
